Was it worn at all - can't find any reference.

Varies between units and i believe it also varies between soldier.
Guy on the right is a 5th Infantry Medic and its abit hard to see but you can see his 5th Infantry Diamond on his left arm.

You see the unit insignia on overcoats, but mostly with units that were still state-side.
You do see them on pictures in the ETO sometimes, but not very often.
