I have set this up in MS Access, as it's quick, can enforce data entry , has good refernetial integrity and I know how to do it . It's poor at websharing, but we'll need to keep it off the web anyway for security .

From this we can produce reports of which players played when ; when they last played, is their kit suitable , we can also manage their AFRA membership , and also CiAs. We can also mail merge off this DB , either totally or selectively.
Is there anything else we want to capture in this initial form ?
Looks good. I'm glad you are doing it - I've spent a fortune on databases over the years with good intentions. MS Access is such a disgusting thing to look at I have only ever done one project using it! I only use Filemaker now - it's pretty and quick. Oh, and MySQL is the work of the devil too!
Is it easy to add or increase fields once done - I used to find that a problem way back in MS-DOS days...
Been using Access for a long time , probably should have migrated to SQL by now 
, but most of my stuff is stand alone . Access has been left behind a bit as it struggles with lots of multi users (it locks too much of the record structure) although you can web host it if you need to .
Adding fields later is not an issue , just means you have to go back and ammend queries and reports , it's no real work , not considering the numbers involved (under 1000)
I can also build a monkey friendly pretty front end onto it if others want to have a go later on . 
I suggest I can build V1 of a memebrship form , which I could send to you to make better looking and then host it on our website as a downloadable PDF/DOC for email back to me? Or we could go posh and try online data capture ... 
OK, I assumed it would be written records from the game day entered into DB later on - but I suppose there might be value in asking chaps to fill a form in pre-game, in anticipation.
I could do a PDF form that can be filled in live and data emailed back - it might be a sledgehammer to crack a nut though.

Not everyone will have PDF editing capability. Web form is a better idea, but surely you'll need signatures for DPA compliance.
They just fill the form in using Acrobat Reader - when the form is returned the data entered is extracted and sent, not the form itself, similar to a web form. You can put whatever guff you like on the form, it's only data entered that gets returned.
There is no need for a signature for DPA compliance but it does prove the person was physically at the game so yes, an electronically pre-filled form will ultimately need to be signed.
Heh, thinking about it, I've got about 30hrs CiA business to do before Eagle so sorting electronic forms and data exchange is going to be impossible. I think handing out a bunch of forms for peeps to fill in on the day will have to do. More sophisticated methods will have to wait!

OK, here is the PDF based membership form.
If you open it up you should be able to type into each field, put ticks over the games you have played and click the submit button. This should fire up you email application with the address already filled in and the form attached to the email.
I'd be obliged if you could give a go please for testing purposes.

The submit button didn't work on my mac using Entourage (Outlook) or Mail.

Have you got the latest version of Acrobat Reader?
Macs don't use Acrobat for PDFs, they use Preview which is packaged with OSX.

There you go - Preview is utter pants. Install proper Acrobat Reader and you will get all the functionality you are missing!

Just tried it on my mac using v7 Acrobat (latest is 8.1) and alls well.
At the moment it just transfers the form data as-is but at some point I'll get it to export a csv so you can bring it into the db Guy. I don't have time this week!
