What is the email address it has to go to ?
Open the form; It says there "To be completed and either submitted via email or posted to: *address* ", and in the top right hand corner of the form there's a big purple button that says "Submit Form" which'll open up Microsoft Outlook and submit it (if you use Outlook), if not, the email is shown in the Outlook window.
